What is an electronic medical record?
An electronic medical record (EMR) is a patient’s chart maintained by their medical provider in digital form. These are essentially the chart notes and medical records that providers use to keep on paper files, but now maintained electronically instead.
An EMR is generally a single provider’s record for a given patient, whereas the patient’s electronic health record (EHR) is a more complete picture of their full medical history, and can be accessed and contributed to by multiple medical providers.1
